If we have not seen you before at Cedar Veterinary Hospital, or if you have a new pet you'd like us to meet, please fill out our new client form and present it at the time of your appointment. We have forms on location as well. If you wish to complete this upon arrival, please arrive 5-10 minutes before your scheduled time.

If you are interested in boarding your pet, please fill out the boarding form and present it at the time of drop off. We have this form on location as well, but filling it out ahead of time can expedite the drop-off process.
If you are interested in adopting someone we have here at Cedar Vet, please visit the adoption page to look at any adoptable animals, fill out the adoption form and bring it with you at the time you visit. Be aware that adoption fees may apply.
